Upon purchase of MBI-GS (S) Transform Survey Hosting (Data) licenses, you will receive an email from info@mindgarden.com which includes a login link to your Mind Garden Transform Account where you will admininster the MBI-GS (S) survey via a campaign. Follow the steps to create the campaign, set the campaign options, invite your participants to complete the survey, and monitor their progress. Terms of Use for Transform Survey Hosting (Data)
Transform Survey Hosting is a data license for research purposes only. This license grants you permission to collect and disclose (a) item scores and scale scores, (b) statistical analyses of those scores (such as group average, group standard deviation, T-scores, etc.) and (c) pre-authorized sample items only, as provided by Mind Garden, for results write-up and publication.

For example, with purchase of the Transform Survey Hosting (Data) and when presenting your findings:

• You may share the group's mean scale scores with survey participants and others.
• You may not share item text in publications with the exception of the pre-authorized sample items included with license purchase.
• You may not copy, modify, or paraphrase content from Mind Garden Individual Reports and release that content to your survey participants or others.

Note: this list illustrates some permitted and prohibited uses of the instrument and is not meant to be all-encompassing.

If you are using this instrument for developmental purposes, you need to purchase online reports from Mind Garden (purchase of Individual Reports includes Transform Survey Hosting license).

The instrument items, directions, manual, individual report, group report, and any other descriptive information available through Mind Garden is the intellectual property of the copyright holder and can be used only with purchase or written permission from Mind Garden.

Reports and Services

• Purchase MBI-GS (S) Individual Reports to provide each participant with an interpretive report of his/her scores. Each MBI-GS (S) Individual Report includes one MBI-GS (S) Transform Survey Hosting (Data) license and one MBI-GS (S) Individual Report license.
• Purchase an MBI-GS (S) Group Report to calculate and summarize average scores for a group of participants. Requires purchase of MBI-GS (S) Transform Survey Hosting (Data) licenses or MBI-GS (S) Individual Report licenses (to collect the data) and an MBI-GS (S) Group Report license (to generate the group report).
• We can customize your MBI-GS (S) survey to add informed consent, demographic questions, and other assessments.

Alternative License Types

• Purchase MBI License to Administer to administer the MBI-GS (S) as an online survey via a non-Mind Garden survey system or as a paper/pen survey.
